Multiple adjacent gingival recessions : which technique to use , tunnel or flap ?

چکیده
Aesthetic concerns play a major role in the treatment planning of dental care and in every periodontal practice. Gingival recessions are one of the prevalent problems that require clinicians’ attention because of the patient’s aesthetic demands and the possible health complications, such as dentin hypersensitivity and root caries. The cosmetic expectations of our patients and their dental health are the primary indication for performing periodontal plastic surgery and soft-tissue reconstructive surgery. It is the responsibility of the clinician to understand patient demands for aesthetics, the indications and limitations of surgical treatment, and to acquire the needed surgical skills to perform delicate procedures aimed at achieving optimal aesthetics. These procedures need to ensure the best possible results with minimal discomfort and maximal safety for our patients. Periodontal plastic-surgery procedures are designed to correct defects in the morphology, position, and amount of gingiva. The present debate focuses on the treatment of multiple adjacent gingival recessions with two surgical approaches: the coronally advanced flap (with or without connective tissue graft) and the tunnelling procedure. Each technique has its own advantages and limitations and the two procedures are well documented in the literature. Here, two opinion leaders – Massimo de Sanctis and Ion Zabalegui – share their clinical views and long-term experience to help us decide which approach we should carry out in different clinical situations.

Vista technique and Platelet-Rich Fibrin Membrane for Treatment of Multiple Adjacent Gingival Recessions - 6 month follow-up
Purpose: To do a clinical evaluation of the results of the treatment of multiple adjacent gingival recessions with VISTA technique (Vestibular Incision Subperiostal Tunnel Access) and platelet-rich fibrin membrane (PRF m). Methods and materials: The study included 30 participants (19 women and 11 men) with multiple adjacent gingival recessions on both jaws of Miller’s Class I and Class II. The ...

BACKGROUND This article reports a clinical case with a 3-year follow-up in which a subepithelial connective tissue graft (SCTG) was used with the tunnel technique to treat multiple gingival recessions, and describes a technique used to enlarge the extension of the graft. METHODS A 41-year-old female patient was referred for evaluation and treatment of maxillary multiple recessions. Following ...
متن کاملThe bilateral pedicle flap-tunnel technique: a new approach to cover connective tissue grafts.
A new surgical technique for the treatment of adjacent soft tissue marginal recession is presented. This technique combines the use of a tunnel procedure with double lateral pedicle flaps to cover a connective tissue graft. This approach combines the advantages of the tunnel technique with the increased blood supply and protection provided by pedicle flaps. Indications include adjacent Class I ...

Background and Aim: Several surgical approaches have been used to achieve root coverage. The Subepithelial Connective Tissue Graft (SCTG) procedure has been shown to be a predictable means to treat gingival recession. Semilunar Coronally Positioned Flap (SCPF) is a simple mucogingival surgery to cover the exposed root surface without harvesting the palatal connective tissue. The purpose of this...

The aim of the present article is to summarize current knowledge in terms of the etiology, diagnosis, prognosis and surgical treatment of gingival recession. Whilst the main etiological factors (i.e. toothbrushing trauma and bacterial plaque) are well established, challenges still remain to be solved in the diagnostic, prognostic and classification processes of gingival recession, especially wh...
